Robotics pricing is usually reverse-engineered from dealer carts. Upper-limb prosthetics are the exception: the US government publishes an allowable amount for every component, to the cent, for every jurisdiction, and updates it on a schedule. On 6 August 2026 I downloaded three of those files — the January 2024, January 2025 and January 2026 DMEPOS fee schedules — and pulled all 151 upper-limb prosthetic L-codes out of them.
Unless noted, amounts below are the non-rural allowable for the largest jurisdiction group (22 states and territories including California, New York, Pennsylvania and Washington). These codes carry no rural differential.
Building the arm
A conventional myoelectric transradial prosthesis, coded out:

Two-thirds of a bionic arm is the hand. The equivalent body-powered build — socket L6100 $2,572.24, hook L6706 $528.35, wrist L6620 $467.82, harness L6675 $186.86, cable L6655 $122.62 — comes to $3,877.89. Electrification is a 11.5× multiplier on the same amputation level.
The step function is in one code
Rank the terminal devices and the distribution is not a curve, it is a cliff:

L6880 is 6.8× the single-motor electric hand immediately below it, 21× a mechanical hand and 54× a mechanical hook. Nothing else in the 151-code set jumps like that. Whatever the fee schedule believes about upper-limb robotics, it believes almost all of it about independent finger actuation.
The elbow is more expensive than the hand
L7181, an electronic elbow with simultaneous control, is allowed $48,155.50 — 1.68× the bionic hand. Stack a shoulder-disarticulation prosthesis (L6965 socket system, L7181 elbow, L6880 hand, wrist, power, glove, L6646 shoulder joint) and the total is $104,703.65, which exceeds the $95,081.03 personal lower-limb exoskeleton code we called the biggest robot number in this file last week.
Two smaller findings sit alongside it:
- The level curve inverts once. Wrist disarticulation (L6925, $10,417.12) is allowed 5.6% *more* than below-elbow (L6935, $9,866.92), even though it is the less proximal amputation.
- Myoelectric control is cheap. Across all six levels, myoelectronic control costs only 14.1% to 19.8% more than switch control. The premium buyers associate with "myoelectric" is not in the control system; it is in the hand attached to it.
Children cost more, except when they cost less

Three of five pediatric devices are allowed more than the adult version; two are allowed dramatically less. There is no engineering story that produces both.
And a detail worth knowing before you budget a research build: the charger is allowed more than the battery in all three power families — 6 V $316.08 vs $297.38 (+6.3%), 12 V $751.31 vs $581.50 (+29.2%), lithium-ion $606.56 vs $467.90 (+29.6%).
The two-year test: nothing moved but the index
Here is the number that reframes everything above. Of the 149 upper-limb codes present in all three files, 147 changed by exactly ×1.024 from January 2024 to January 2025, and by exactly ×1.020 from January 2025 to January 2026 — identical to four decimal places, from a $528 hook to a $58,767 elbow. The two exceptions differ only by rounding.
On 5 August we made this argument from three codes and called it indexation rather than pricing. With 149 codes and two consecutive updates, it is no longer an inference. In two years during which multi-articulating hands shipped new grip sets, new sensors and new price points on the private market, not one line of the government's price list responded to any of it. The file is a schedule of numbers being carried forward, not a measurement of what these devices cost to make.
Dispersion, and one artifact
Geographic spread on the flagship code is small: L6880 has seven tiers across 53 jurisdictions, from $28,715.00 to $31,586.51 — a 10.0% range. Ceiling and floor are pure arithmetic on the allowable, ×1.20 and ×0.90.
L7180, the sequential electronic elbow, is the opposite: fourteen tiers, from $39,992.59 to $50,007.15 across the states (+25.0%) — and $18,180.91 in Puerto Rico, less than half the mainland minimum. That figure is not a Puerto Rico elbow price. The identical $18,180.91 appears in Puerto Rico on six unrelated codes (L6955, L6960, L6965, L6970, L6975, L7180), which is exactly the signature of the placeholder values we have been cataloguing on dealer sites — multiple different machines carrying one number. It is worth knowing that government files carry them too.

Limitations. These are allowable amounts, not manufacturing costs and not retail prices; they include the prosthetist's work as well as the device. The stacked builds are my configurations of published codes, not quoted prostheses, and a real episode of care adds codes not shown (test sockets, replacement sockets, evaluations, repairs). Year-over-year factors were computed on the ceiling column, which is a fixed multiple of the allowable, so the comparison is unaffected by jurisdiction. Private payers negotiate their own rates against the same code set.
